Whichever way you decide to apply, you will need a copy of document proving you identity (copy of ID card, passport, birth certificate, or similar) AND proving low-income (social security award letter, W-2, U.S. 1040 income tax form, or similar) OR proving participation in a governmental program (member’s card copy).
